All Sick/Fit Note requests are now being processed online via ‘Rapid Health’. This service is open 8am to 6.30pm, Monday to Friday (except Bank Holidays).

To do this:

1. Select and click the round button below which has the name of the surgery you are registered with

2. Select ‘Questions and Admin’ followed by ‘Sick/Fit Note Request'

3. Complete and submit the form and your request will be actioned
