- Encouraging and supporting long lasting habits and behaviour change
- Using a 'goal setting' approach
- Looking at anything that has blocked you in the past and helping you to move forward

How Do I Get an Appointment?
You will need to see your GP to be 'referred' to the Health and Wellbeing team.
