How Can a Health and Wellbeing Coach Help Me?

A Health and Wellbeing Coach offers support and guidance. They can help you improve how you manage your physical and mental health. This can include people with long-term conditions. Often these conditions can be managed through lifestyle changes.

Our coaches aim to help people make positive lifestyle changes. By setting goals and targets you are more likely to achieve healthier habits. Embracing knowledge and learning new skills will boost confidence. This leads to positive long term behaviour change.

Working with you through a series of sessions, they can support you in taking proactive steps to:

  • Improve the way you manage your physical and mental health conditions. We base this on what matters to you.

  • Develop knowledge, skills and confidence in managing your health and care

  • Help achieve your personalised health and care plan goals

  • Help you work through any 'blocks' preventing you from moving forwards to reach your goals

  • Improve your health and quality of life

 

What Can I Expect From an Appointment With a Health and Wellbeing Coach?

During the first session with a health and wellbeing coach they will find out more about you and:

  • Your lifestyle

  • Values

  • Goals

  • Anything that might get in the way of achieving those goals

You will then set some short-term goals that will enable you to start to work towards what it is you want to achieve.

In the sessions that follow, you will:

  • Review your progress

  • Set new goals

  • Discuss any challenges you might have faced to help you overcome these in the future

In doing so, health coaching helps you develop the skills and confidence to manage your health and wellbeing.

Will I Need to See a Health and Wellbeing Coach On a Regular Basis?

You will be offered 4-6 sessions of support. Each consisting of up to 60-minute consultations by either:

  • Phone

  • Online

  • Face-to-face

The amount, frequency and duration of appointments is determined by the patient and their goals.
