What Does The Care Home Assessment Team (CHAT) Do?
We are proud to have our own 'Care Home Assessment Team' (CHAT) here at Coastal Medical Partnership.
This means each of our care homes has a named clinical lead and receives a weekly ‘home round’ with residents.
Residents have a person-centred holistic health assessment of need. This can include physical, psychological, functional, social and environmental needs. If required, a resident will also receive a structured medication review.
