Our team visits residents in their care home. This is part of the weekly care home ward round or on an individual basis as needed. They are able to assess, diagnose and treat complex physical and mental health.

The team aims to maximise opportunities to enhance the quality of life of residents. We involve residents in the decisions about their medical care.

We provide:

• Face to face assessment/telephone triage/video calls
• Rapid visits at time of acute crisis
• Treatment and symptom management for sudden illness or conditions
• Falls assessments
• Palliative and 'End of Life' care
• Advanced care planning
• Medication reviews
• Long term condition reviews and management
• Health education and health promotion advice
• Assessment and treatment advice for common mental health concerns including dementia, depression and anxiety
• Education and training for care home staff
• Urgent blood tests/ECGs
