What Does The Proactive Frailty Care Team (PFCT) Do?

We are proud to have our own 'Proactive Frailty Care Team' (PFCT) here at Coastal Medical Partnership.

This means our housebound patients may receive a home visit by a named clinical lead.

Housebound patients will have a person-centred holistic health assessment of need. This can include physical, psychological, functional, social and environmental needs. If required, a housebound patient will also receive a structured medication review and an Advance Care Plan.
