What Does The Proactive Frailty Care Team Do?
Coastal Medical Partnership offers home visits for patients who cannot come to the surgery because they are housebound. A Nurse Practitioner from the Proactive Frailty Care Team will visit you at home. The visit may take up to 2 hours.
The aim of the visit is to learn more about your health, wellbeing, and the support you may need. This helps us make sure you get the right care.
During the visit, the nurse may:
- Check your general health and talk about any long-term health conditions you have.
- Review your medicines to make sure they are still right for you and are working well.
- Talk about how you manage everyday activities, such as moving around safely, keeping your balance, and looking after yourself at home.
- Check your memory, thinking, and concentration if this would be helpful.
- Talk about the support you receive from family, friends, carers, or other services.
- Discuss your wishes for future healthcare. This is called Advance Care Planning. It helps healthcare professionals understand what is important to you if there comes a time when you cannot tell them yourself. This may include a ReSPECT form and discussions about emergency treatment decisions.

Who Can Have a Home Visit From The Proactive Frailty Care Team?
This service is for patients who are genuinely housebound and unable to attend the surgery.
Home visits are arranged based on a patient's health needs and whether a home visit is the most suitable option. We will contact eligible patients over the coming months to arrange an appointment.
Can You Have Someone With You At The Visit?
Yes, if at all possible, we actively encourage you to have a family member, friend, or carer with you during the visit.
Someone who knows you well can help answer questions and support you during the assessment.
