What Does the Learning Disability Team Do?

Here at Coastal Medical Partnership, we are proud to have our very own Learning Disability (LD) Team.

As a team, we are here to support and improve the health and wellbeing of those patients with a learning disability. Each year, we will work with you to create a health action plan. This plan will outline your health goals for the next year. We do this through completing Annual Health Reviews. The reviews are a great opportunity for you to discuss any concerns or worries you have with a clinician.

Who's Who in the Learning Disability (LD) Team

GP Lead: Dr Chad Tew

Nurse Lead: Michelle Harford

Nurse: Debbie Taylor

Care Co-Ordinator: Danielle Buckland-White

There are also 'LD Champions' at each site within Coastal Medical Partnership. They can offer advice and support if any of the above are not available.

Lead GP

A Coastal Medical Partnership GP who provides leadership for the Learning Disability Team. They are an advocate for people living with a learning disability within our surgery, Primary Care Network (PCN) and community.

Lead Nurse

The Lead Nurse works closely with LD patients and families. They give:

  • a holistic approach to health and social care needs
  • build therapeutic and professional relationships with patients, family and staff
  • complete a yearly annual health review, building rapport and familiarity
  • liaise with outside and wider agencies where necessary and make health and social care referrals

Care Coordinator

The Care Co-Ordinator provides a point of contact for our learning disability patients and gives help and support to patients and family members. This includes anything from:

  • health queries
  • helping you find local activities
  • making referrals when needed

The Care Coordinator also helps to create the Health Action Plans and ensures each patient or carer receives one.

Learning Disability Champions

Available at each site when the above are not available to answer queries and offer support.
