A Health and Wellbeing Coach offers advice on the best way to access help for patients feeling low, isolated, anxious, stressed or depressed. Helping create plans to successfully achieve long term changes and connect with local services that offer support.

What is a Social Prescriber?

A Social Prescriber works with patients focusing on ‘what matters to me’, taking an holistic approach to people’s health and wellbeing in order to connect them to appropriate community groups and services.

What is a Mental Wellbeing Advisor?

A Mental Wellbeing Advisor works with other health professionals and undertakes assessments of patients to understand their individual needs and identify the most appropriate support for them.

What is a Peer Support Worker?

A Peer Support Worker works with other health professionals to help support patient wellbeing and provide inspiration for their recovery.

What is a Mental Health Practitioner?

A professional with mental health expertise. They support people with mental illnesses and can help people to:
  • Build skills to manage their own mental health
  • Live well in their communities
  • Connect with other services and access the best support for them
